How did Alex get out?

I always figured that the "nightmare" at the beginning was actually a distorted version of Alex's escape from the hospital. He was about to get some electro-shock therapy, he got loose, found a knife, and killed a lot of nurses on the way out. Considering his condition, the altered world isn't too far of a stretch.
