I always wondered what Alex meant when he said that to his mom. You know, the first time you encounter her in the game, after you take the gun off her lap? Well, my question is- what the hell does he mean? There didn't seem to be anything wrong with her dress.

I also believe that he was going to comment on her dress being wet. The wet spots on the floor leading to her would imply she was at least a little wet.

It's more like a slime trail
I think the trail of fluid is suppose to represent amniotic fluid (it looks thicker than water in this instant) to fit in with Alex's mom's theme of motherhood, pointing a finger to the final boss fight.
^ Or maybe... Alex's mom gave birth to the lurker, who crawled to the basement and after seeing what kind of creep borned she went all nuts and decided to kill herself?

she seems very hurt by the fact josh is dead, and that she couldnt be close to alex.

Protection? The woman is oblivious to the world around her. It is Adam's old pistol. Adam has gone out to "find Josh". It's really entirely possible she's suicidal, in a state of denial about Josh's death, the same as Alex is, but feeling a horrible depression over her, a sense of despair she can't ever shake,

Same logic can be applied for the idea of her being suicidal. What is she gonna do? Pistol-whip herself until she dies?
I was under the impression she is suicidal to some extent too though. Having a revolver in her lap while talking about how much she misses her dead son didn't help the matter any.
