Recently. Not centuries, or even decades, ago.

Brookhaven Hospital opened in the 17th century. Unless in 400 years only two people, Ernest Baldwin and Laurence Monneret, have been considered the hospital director, it's highly unlikely to be a painting of him.

Brookhaven Hospital opened in the 17th century.
Indeed. If I'm not wrong the Hospital was opened due to a plague.

The picture you're talking about, Animal, it even says that it's a picture of the director of the old Brookhaven Hospital.

I have a question.
About Baldwin being Joseph Barkin's doctor: I thought that the three patient records (Jack, Joseph, Joshua) were symbolizing James, 'cause of their name starting with the letter "J" and his three sides/symptomes: suicide tendencies, guilt, violence.

And how did you come to the conclusion that the "HOLE" message was Barkin's and that even Baldwin was his doctor?

They do symbolize James, but they were real people. Just like Eddie and Angela symbolize fates James could end up having.
