I thought I was a pretty good SH fan but I guess I'm a lil rusty on the history. This isn't a spoiler as you encounter it right at the beginning of the game. My question is why is the hospital different in layout?

No it's not. The only difference is an additional hallway on the first floor that leads to a rear exit.
