not all the monsters were, just that one i think. douglas was following heather and would of asked why she killed humans. douglas doesnt see the exact same thing heather does.

Claudia never says anything like that. It's discussed in "Silent Hill Chronicles," but Claudia never explicitly states "I've transfigured these people to look like monsters in order to piss you off."

i wasnt sure, but if they were, they would be attacked by monsters ( whether they looked different to them or not)

Aside from the Missionaries, all the monsters that Heather sees don't exist for others. Vincent, for instance, wouldn't see them. And he doesn't.

yeah but i think he knows whats going on. remember the whole you see them as monsters joke thing? either he guessed, or he knows something.
but this proves that heather only sees the monsters, becuase vincent and anyone else doesnt get attacked.. by the monsters.

Do you mean how she can exist in two realms or.. how she can move between the two? For the latter: people have been moving between these realms all the time in the Silent Hill games. That's an effect of the town, and the power of it. As for the former query I don't believe anyone's trying to say she exists in two of them at the same time.

She doesn't. From what I understand, she (and All the Protagonist) exist in or the other. Not both at the same time.

She told Heather that she merely gave the order to the Missionary to execute Harry. You need to pay attention to what's being said in this game.

Higher ranking cult members see the 'otherworld' too, right? I think I recall Vincent saying he finds it "fascinating."

Vincent definitely seems to see the otherworld. However, remember that people can be in the otherworld, and see different things at the same time. Remember James, Eddie, and Laura from SH2.
As for the monsters, cultists, and Heather:
What it seems to be is that Heather and the monsters are in one Otherworld, and the cultists, Claudia, Vincent, and Leonard are in another, without the monsters but a similar world. They can all see Heather and she can see them, and Leonard and the Cultists appear to her as monsters.
