Why did Cybil's death have to be canon according to SH3 and why did Harry have to die?

Who stated that Cybil died in the first game?...it's a possible outcome, but i don't think it was the canon outcome. I can't remember anywhere in SH3 that it made a reference to Cybil dying.....
