You can actually see where it lays .Its on your right side after you drop down to the battle.

The rifle is in the same room with the 2nd boss. I'd say it's difficult to miss it.

No. This one is easily avoided if you use the blood pack, it's the one that Travis pointed out in his last post.

Ahhhh,noooow I get it.I´m so dumb sometimes.
Its really easy,but my answer depends if you want the answer,or if you don´t want to get the answer spoiled.

Here is the way to solve it:

In this hall should be two plates,one with a list of persons and their dates they are born at and died,and another one with the riddle.You must put the names into the right order (I don´t know anymore if you should sort them after their age,or when thy died.).Then you should get a "special" word already mentioned.

Lol, finished the game like 2 months ago. Finished 2 times. When I got to Nowhere with the Grim Reaper's puzzle I kinda remembered about the Alert thing that you said and voila! Puzzle was gone in like 2 minutes

As for the valves' puzzle, you have to turn the left valve left 2 times and the right valve right 3 times.

For future reference (I'm not sure if this has been posted elsewhere or not, sorry if it has) I turned the right valve left twice, then turned the left valve right once and it worked. Not majorly important, but requires less turns.
